Online Forms

At Old Saratoga Eyecare, we offer you the ability to print patient forms and complete them in the convenience of your own home or office. You can return them to the office in person, by mail, or via fax. We ask to have all forms completed and returned to the office one week prior to a scheduled appointment in order to proceed as scheduled.
